Treatment costs will be discussed and financial arrangements can be made.    Please complete the new patient forms and bring them with you at your first visit. Also, if you have dental insurance and bring all documentation (cards, policy numbers, phone numbers, etc.) we can bill your insurance company for you.    If you have any questions, please call at your convenience.
